Don't know your Department of Labor number (Employers DOL #)?   Call 586-8914 with your Federal Employer Identification Number and they can tell you.

Mail the completed ETF forms to the Local Branch Office.  On Oahu that is:

They should arrive at least 2 weeks prior to the class date!

Do not send your part of the payment to the Workforce Development Division.
